Any dead ideas?

Matt Miller's book, The Tyranny of Dead Ideas, makes a simple but profound point: that until we collectively let go of cherished and outmoded ideas (such as the notion that every generation will out-earn the previous one) we are doomed to make poor long-term decisions.  What dead-but-durable ideas hold educational institutions back?  Any candidates for the list?
